Organization providers (Type 2 NPIs)

CMS assigns an Entity Type 2 NPI to an organization that provides health care, such as a hospital, group practice, clinic, pharmacy, laboratory, nursing facility, or home health agency. A large organization may hold several Type 2 NPIs for its parts, and the people who work there hold their own Type 1 numbers.

What an organization profile shows

Each Type 2 profile here displays the legal business name recorded in NPPES, the NPI, the entity type, the active status in the snapshot, the primary practice location and practice telephone as reported to NPPES, the NUCC taxonomy codes with their official display names, and the enumeration and last-update dates. Authorized official names, mailing addresses, fax numbers, employer identification numbers, and other identifiers are never published.

What the number does not tell you

An organization NPI does not indicate accreditation, ownership, the services offered, network participation, or whether the location is open. Verify the current record in the official NPPES NPI Registry and confirm details with the organization.
