Individual providers (Type 1 NPIs)
CMS assigns an Entity Type 1 NPI to one human being who provides health care, for example a physician, nurse, pharmacist, therapist, or dentist. The number belongs to the person for life, regardless of where they work or which organizations they join.

What an individual profile shows
Each Type 1 profile here displays the name and credential text as recorded in NPPES, the NPI, the entity type, the active status in the snapshot, the primary practice location (street, city, state, and ZIP code) and practice telephone as reported to NPPES, the NUCC taxonomy codes with their official display names, and the enumeration and last-update dates. Mailing addresses, fax numbers, license numbers, and other identifiers are never published. Individuals who want their practice street or telephone withheld from this site can use the corrections and suppression process.
What the number does not tell you
An individual NPI does not confirm licensure in any state, board certification, hospital privileges, insurance participation, employment, or whether the person is currently seeing patients. Those facts live with state licensing boards, payers, and the provider. Verify the current record in the official NPPES NPI Registry.
