This directory uses exactly one data source: the public NPPES data dissemination file published by CMS.
The NPPES data dissemination file
The National Plan and Provider Enumeration System (NPPES) is the CMS system that assigns National Provider Identifiers. CMS publishes the full NPPES public file every month, with weekly incremental updates, on its NPI Files page. The file contains the data elements that providers report when they apply for or update an NPI and that CMS designates as disclosable under the Freedom of Information Act.

What is used and what is not
Only the main provider file inside the archive is processed, and only the columns listed on the methodology page are read. The other-name reference file, the practice-location reference file, the endpoint reference file, and the weekly incremental files are inventoried but not used. No other data source, commercial database, licensing-board file, or user-submitted information is combined with the NPPES data.
Taxonomy codes
Health care provider taxonomy codes are maintained by the National Uniform Claim Committee (NUCC) and published in the Health Care Provider Taxonomy code set, which is copyright American Medical Association. This site displays each code exactly as it appears in the NPPES file together with the code set's official display name and its grouping, classification, and specialization from version 26.1. Code definitions and notes are not reproduced. The code set is available from NUCC.

Official lookup and updates
The authoritative, continuously updated record is the NPPES NPI Registry. Providers update their own records through the NPPES portal.
